Connecticut Home Care Program for Elders

The Connecticut Home Care Program for Elders (CHCPE) is designed to help senior citizens stay in their home and not be unnecessarily institutionalized.

Who qualifies for this program?

  • Anyone 65 years or older

The Department of Social Services has divided CHCPE into several categories based on financial and function eligibility

  • Category 1: Individuals at risk of being hospitalized or placed in a nursing facility if home care services cannot be provided. This person must require help with one or two critical needs
    • There is no income limit for an individual in this category. If you are eligible for Medicaid coverage in a nursing home you are eligible for home care services.

 

  • Category 2: Individuals who are frail enough to require nursing facility care, but who are not considering nursing home placement at this time or who have resources that would prevent them for qualifying for Medicaid if they were to be placed in a nursing home. This person must need help with three or more critical needs.
    • There is no income limit for an individual in this category. If you are eligible for Medicaid coverage in a nursing home you are eligible for home care services.

 

  • Category 3: Individuals who need long term nursing home care funded by Medicaid. This person must need assistance with three or more critical needs
    • There is an income limit for an individual in this category. The person can’t make more than 300% of the Supplemental Security Income limit. (In 2016 this was $2,199.00 per month for one individual)

 

  • Category 5: Individuals who are at risk of hospitalization or being placed in a nursing home if home care services are not provided. This person must need help with one or two critical needs
    • The income limit for this category is $1,437 for an individual.

 

As part of the eligibility, an access agency will examine whether the applicant’s needs can be provided for at home for less than the cost of nursing home placement. The cost limit for each category varies and can increase in response to individual need.
